How this works:
 

• Get your letter at the end of your virtual appointment.
   

• You do need a quick virtual appointment so a provider can legally produce a document that affirms your appointment. 
   

• Forging this document is illegal and unethical.

If you are already an existing patient, this is included with your visit.


If you are not a patient and only want a letter:

 

• You must fill out this "Registration Letter Only" prior to your visit.

​

• Cost is $100 for 1 visit and letter, fee is due when you self schedule.

​
