This visit includes a review of your medical and social history related to your health and education and counseling about preventive services. This personalized prevention plan is designed to help prevent disease and disability based on your current health and risk factors.

  • Health Risk Assessment Questionnaire

  • Review Medical and family history

  • Review list of providers and medications.

  • Order Screenings and referrals if needed.

  • Get flu and pneumococcal shots if needed.

  • Height, weight, BMI, and blood pressure measurements.

  • Detection of any cognitive impairment. Signs of cognitive impairment include trouble remembering, learning new things, concentrating, managing finances, and making decisions about your everyday life.

  • A simple vision test.

  • A review of your potential risk for depression and your level of safety.

  • An offer to talk with you about creating advance directives. (See Blog Article)

  • Personalized health advice.


Vaccines

Flu - Yearly

Pneumococcal - (2 total) 11+ months apart

Hep B - If risk

Bone Density Screen- DEXA

Women 60y+,

every 2.5 yrs **


Breast Cancer Mammogram

40+y, 1-2 years. **

21-65 years

Every 3-5years**


Colon cancer screen

45-75y

Every 5-10years **


Lung Cancer (LDCT)

55-77y, smoker or w/in 15yr, 30 ppk


Prostate Cancer Screen (PSA)

50- 75year.

Yearly


Diabetes

If abnormal, every 3 months


Lipids - Cardiovascular Screen

At least every 5 years** (Recommend yearly)


Glaucoma

Yearly


Hep C

If born b/t 1945-1965 or IV drug user

Once at least.


HIV

If at risk


Other Screenings

Alcohol, Tobacco, Nutrition, Activity, Depression, Safety.
