You are responsible to make sure you are covered the day of your visit.

Insurance will be filed on your behalf for your convivence, however if denied, you will be responsible for payment.  **Make sure to update the clinic if insurance changes asap.

Addiction = PPO & Medicaid.  Medicare with referral.

Primary Care = Limited. Only PPO, some exceptions accepted.

  • $200 for Follow Up visits

  • $100 for Quick Physical
